Full Job Description
Are you a Band 5 Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 5 nurse to support the changing face of community health care within our Rapid Response Team. As part of the Urgent Community Response and Virtual Ward initiative, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect.
These posts will be to cover Mid Bedforshire meaning there will be the expectation the applicant can flex to which area had the most demand and need.
Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions., You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community.
Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Consideration will also be given to staff who are newly qualified and are passionate about a career in community nursing. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.
Our mission is to make a positive difference to people's lives by improving the quality of life for all we serve. Our values are: We Care, We Respect, We are Inclusive - so we are looking for people who live and breathe these qualities when supporting service users and carers, and in their relationships with colleagues in the Trust and our partner organisations
