Community Psychiatric Nurse - Older Adults

An exciting opportunity has arisen for an innovative role of Mental Health Practitioner with the Older Adult Mental Health team Brecon, Powys.

This role would also provide an opportunity to be mentored or to mentor, career progression and continued education.

We are seeking applications from Registered Mental Health Nurses, Social workers or Occupational Therapists, who have experience of working in community services., Responsible for the specialist assessment and care of clients and provide relevant appropriate treatments to allow them tomaintain independence for as long as possible in the community. Liaise with specialist services within their catchment area and develop links with Primary Care Services and other caregivers.Manage outcomes autonomously; link to peer support through supervision and video conferencing facilities.
