Full Job Description
The enhanced team west is looking for a registered mental health nurse who is enthusiastic in wanting to support people and their families/carers in a holistic way in the community, for those people who have complex needs.
The successful candidate will offer a person-centred approach to promote independence, well-being and support for people to live a fulfilling life in the community., The successful candidate will maintain a high standard of clinical work when carrying out nursing procedures.
The successful candidate will have a caseload of people with individual care plan and positive risk management with the support and input from specialist practitioners.
This will include being responsible for the safe custody of medication, including checking, administration and transportation of medication.
Ensure compliance with the Trust policy and procedure and clinical governance framework.
Knowledge and compliance of the Lone working policy.
Working in a multi-disciplinary team and having excellent communication skills with team colleagues and other services.
The team is a seven-day service; therefore, the candidate will be willing and agree to work some weekends.
Participate in the role of team duty, to include delegation of daily tasks and supporting the team flexible assertive community treatment approach (FACT).
Champion service user safety and dignity and drive continuous improvement.
