Full Job Description

We're looking for an experienced CPN ready to hit the ground running and manage the complexities of a shared community caseload in Maidstone's Mental Health Together to cover staff leave. You will be required to work autonomously, with the support of peers and Leads, to ensure the safe and effective care of clients sat within shared caseloads. You will also be required to cover duty on a rota. The environment is fast-paced and highly unpredictable; therefore a level of resilience and organisation is required.,

  • You will carry out and coordinate comprehensive, systematic nursing assessments and reviews which take account of relevantmental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with the service user and their network, to agree a holistic person centred care plan that addresses the individual's needs.
  • The post holder will need to undertake crucial screening and assessments, facilitate appropriate referrals to mainstream primary and secondary mental health services, and act as an advocate for clients in the referral process and offer continued support and brief interventions with a view to maintaining/enhancing engagement.
  • To act as a leader, supporting best practice in line with NICE guidance.
