Full Job Description

The Community Mental Health Service (CMHS) in Bracknell has an exciting opportunity for a Band 5 Community Psychiatric Nurse to join our dynamic and multi-disciplinary team to provide advice, support and treatment to people and their carers. The post holder will manage a caseload of clients receiving regular, injectable anti-psychotic medication (depots) and carry a small caseload as a 'named worker'.We are a joint funded team that provides both health and social care to support individuals aged 18 years and above with complex and severe mental illness. The service runs Monday-Friday, 9:00am - 5:00pm.We are looking for dynamic candidates who are enthusiastic and passionate about patient care and recovery., o To have clinical responsibility for the 'depot caseload' as specified by the Clinical Team Lead.o To assess service user needs, plan effectively, monitor and evaluate interventions using evidence based practice which is relevant to individual needs and within a framework of research and audit.o To participate in duty work on a regular basis.o To establish and maintain good liaison with GP practices, and other physical and mental health services including other relevant agencies and organisations in the area.o This post is not for usual care co-ordination but there is opportunity to begin learning about the role of the 'named worker' under the new CMHS model.o Support of nursing students.
