Community Psychiatric Nurse

The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant to the care group to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students.,

• To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
• To support/manage a team caseload and attend relevant MDT meetings
• To regularly support and cover the duty rota
• To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
• To provide risk screening and assessment as well as risk management planning
• To contribute to and facilitate evidence-based assessments and individual and group interventions
• To contribute to the maintenance and development of the Service
• To regularly share and supervise students on practice placement, and junior staff as appropriate
  
  Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
• Completion of Mentorship Course/ENB equivalent
• Evidence of continuous professional development, Significant post qualifying experience in the relevant care group
• Experience of working alongside people with complex health problems in the designated care group
• Experience of supervising staff
• Experience of managing a caseload
• Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
• Experience of working consultatively with professionals
• Experience of conducting clinical assessments including risk assessments, Successful candidates are normally appointed to the bottom of the pay scale unless previous NHS experience in a similar role can be evidenced., You must have appropriate UK professional registration.

Our service is in the process of moving away from a more traditional model to adopt the follow me care principles advocated in NHS England's new Community Mental Healthcare Strategy. We are confident that this approach will help us to meet the growing demands for services whilst continuing to provide good outcomes for people. This post will sit within our multidisciplinary community dementia team and will work as a lead practitioner to deliver interventions to those people who have more complex or intensive needs. You will also be required to work in Intensive support which is also part of the DOPMHS team so flexibility is key. We aim to help people to remain safe and well at home and able to maintain the relationships that are important to them. The service has an intensive support element within it that offers extended weekend interventions to people who are seen by the team. The service operates 0900 - 1700 hours Monday to Friday. The Intensive Support also work weekends and bank holidays.
