If so, this role may be for you.
TheUrgent Community Response Frailty Hospital at Home (UCR FH@H) team delivers hospital-level urgent and same-day responses to people in their usual place of residence following a sudden deterioration in their health or condition. This could be a one-off visit or a short episode of care, delivered through multidisciplinary team working and working collaboratively with other community services. The timely responses and high level of holistic assessment and effective clinical interventions reduce the need for unnecessary hospital conveyances or admissions and enable patients to remain at home where they recover best.The post holder will work with a range of professionals to ensure care is appropriate and delivered in a timely manner. The post holder will be able to demonstrate their professional skills and competence in leadership, care planning / co-ordination, and care and treatment delivery.
Main duties of the job
The post holder will respond to clinically appropriate 0-2 hour and 2-24 hour community response referrals, alongside planned follow up visits across the breadth of the service, providing holistic assessment and delivering hospital level treatments and care in the patient's usual place of residence.
Assess physical and psycho-social needs and provide interventions tailored to meet the needs of the individual based on best available evidence
