North Tyneside Recovery Partnership have an exciting opportunity for a Band 6 nurse to join the team, working into the Physical Health Pathway

The role entails working with those affected by substance & alcohol misuse with associated and additional physical health needs.

The role entails providing safe and effective interventions, harm reduction advice and use of psychosocial interventions to support service users to consider and achieve recovery.

The role involves individualised clinical assessment, care planning, formulation, and collaborative working across the multi-agency team.

The team consists of a well-established mix of professionals that work collaboratively to deliver individualised care.

The applicant will be expected to demonstrate leadership within the team, providing supervision and supporting students.

Main duties of the job

To be responsible for the Assessment and Management of casework of service users who have substantial complex & physical health needs.

Provide physical health wellbeing checks including taking of bloods, observations and having an outreach approach

Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.

In collaboration with the Service User & Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.

To deliver evidenced based Clinical and Psychologically based Interventions.

The post holder will work as a member of the MDT ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.

Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.
