Full Job Description
The Surrey Downs Health and Care Community Therapy Team works across organisational boundaries within the Primary Care Networks, and is responsible for providing therapeutic/MDT intervention for those who would otherwise need emergency hospital admission, as well for as those medically stable to return home from an acute hospital but requiring enhanced short term support as one stage in their on-going rehabilitation pathway. The Surrey Downs Health and Care Community Therapy Service innovative rehabilitation for adults providing joined up, co-ordinated care to local citizens with health and rehabilitation needs. The service aims to support people living as independently as possible by offering multidisciplinary assessment and further rehabilitation. The Primary care network provides a multidisciplinary approach to support the individual and facilitates a holistic assessment within the framework of the MDT and treatment response with the purpose of reducing risk admission to acute system and providing individualised, targeted rehabilitation. This role will be based within the Cobham and Oxshott PCN. · To perform advanced physiotherapeutic assessment of the patient with complex presentations using advanced clinical reasoning skills to create targeted treatment plans using SMART goals specific to the patient's condition. · To formulate and deliver an individual or group physiotherapy treatment programme In line with best practice guidelines. · To help co-ordinate and ensure effective delivery of therapy services liaising and co-operating with colleagues across other PCN's/Adult social care teams to provide equitable services. · To evaluate patient progress, reassess and alter treatment programmes as required and to advise and support more junior colleagues to this effect. To handover treatment plans to the rehabilitation assistant to support with the patient centred goals. · To have an awareness of local community services and be able to refer to these appropriately and in a timely manner. · To understand and manage clinical risk within own patient caseload and within the service and advise more junior staff, seeking guidance from Band 7s/Clinical Lead as required. · To keep accurate and complete records of work undertaken in line with CSP, Community Therapy Team, Surrey Downs Health and Care standards. Maintaining confidentiality and information governance standards at all times. · To teach, assess and contribute to the performance assessment of Band 5's and Rehabilitation Assistants. To include support less experienced members of the team · To participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services to the local population. · To promote effective teamwork delegating activities appropriately, to the benefit of the patient and service delivery. · Responsible for the provision of Physiotherapy service for Surrey Downs Health and Care. · To liaise closely with other health and social care professionals. · To supervise and teach less experienced Physiotherapy staff, Rehabilitation Assistants and students within this speciality. · To contribute to audit, implementation of best practice guidelines and contribute/participate in in-service training
