Full Job Description

We are looking for a thoughtful, proactive and enthusiastic individual to join the friendly team within our Integrated Community Services Team in Scarborough. The role would suit either a newly qualified Physiotherapist or a Physiotherapist looking to gain Community experience. As a community Physiotherapist you will be working with the Therapy Team, assessing and treating patients referred by all our health and social care stakeholders, providing a high standard of care and physiotherapy services to patients with diverse and sometimes complex conditions in their own homes and care homes. Because the patients are community based it is essential to have a car and driving licence for the UK. You will be responsible for providing specialised physiotherapy assessment, treatment and advice to an independent caseload, working closely with colleagues in this multi-disciplinary team and supported by senior Physiotherapists. This post may develop into a rotational post across Humber Community Services. We are committed to our Trust values of caring, learning & growing & offer a variety of learning opportunities with regular supervision and in-service training programmes., As a Community Physiotherapist, you will have access to supportive supervision and peer review from a well-established team of Physiotherapists. Within this role you will:

  • Perform comprehensive physiotherapy assessment of patients with diverse presentations, using clinical reasoning skills and manual assessment
  • Formulate and deliver specialist individual physiotherapy treatment programmes based on a sound knowledge of evidence-based practice
  • Formulate accurate prognoses and recommend best course of intervention
  • Evaluate patient progress, reassess and alter treatment programmes if required
  • To provide line management, supervision, support, guidance and training to junior staff and students
