Are you looking for a place to work that will offer you support and the opportunity to develop and embed your physio skills?
Come and work with us in Swadlincote Integrated Community Team where we have a well-established MDT and can give you the opportunity to see patients with a wide range of multi-pathologies.
The therapy team is comprised of Physiotherapists, Occupational Therapists and Support Workers who work closely with the wider multi-disciplinary team to provide rehabilitation to patients in their own home.
This post is an excellent opportunity to develop your skills and knowledge working within supportive environments with access to a preceptorship programme., Our Integrated Community Services Teams are enthusiastic with a breadth of experience and a shared goal to offer the highest standard of care to our patients.
All team members are expected to carry their own clinical caseload, attend regular therapy meetings and undertake individual and joint therapy assessments and treatment sessions with members of the MDT where appropriate. Previous post qualification experience is desirable but not essential.
Rehabilitation is delivered to patients in their own homes with a focus on improving health and well-being, working toward patient-centred goals and preventing hospital admission or facilitating discharge home.
You will be able to develop your knowledge and experience around Falls and Frailty and Triage.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
