Full Job Description
The post holder will work independently under the direction and guidance of a registered nurse in order to provide efficient and effective evidence based nursing care to patients in their own homes. He/she will work in partnership with team members, service users, general practice staff and other agencies.
To work as part of the District Nursing Service and other stakeholders to provide efficient and effective nursing care as delegated to patients in their own home
Band 3. Community Phlebotomist
In Croydon, Health and Social care providers and commissioners have formed the Croydon Alliance Board with the aim of providing a whole system transformation that will deliver the outcomes our over 65's have specified that they want.
The Outcome- Based Commissioning programme is a radically different approach to funding and delivery of services designed to deliver outcomes local people want whilst getting best value from the Health and Social care sectors. This includes the provision of well-coordinated care and support to promote independence in the most appropriate setting.
We require a Full Band 3Community Phlebotomistto work within the Community Nursing Service to provide efficient and effective nursing care to patients in their own homes. The ideal candidate should have experience working within a Health Care setting, be compassionate and able to communicate effectively with patients.

All candidates must have a clean driving license and use of a car for work purposes. Once recruited staff can take advantage of our child care facilities.
