Full Job Description

The post holder will be responsible for a patient caseload, with opportunities to oversee and manage their care. The post holder will be supported by and line-manage a well-established, skilled team, including, Senior Community nurses, Nurse Associates and Healthcare Support Workers.

The post would suit an experienced Community Nurse who has undertaken prior caseload management and has a passion for providing excellent patient-centred care. Support is available onsite from the Nurse Lead who oversees the North Neighbourhood team.

As well as your integrated care colleagues, you will have access to and support from the wider expertise of our specialist teams including Evening and Overnight Nursing team, early supported discharge team, Rapid Response, Palliative Care, Tissue Viability, bladder & bowel, community stroke and neurology team, respiratory, heart failure and diabetes teams as well as the in-patient wards: who all aim to work very collaboratively.Your role will include working closely with our GP colleagues and practices.Benefits of working as a nurse for CNWL Camden:o Training and career development opportunitieso Opportunities for research and audit to support innovative practiceo Open to flexible working patternso Good opportunity for progression

The service operates from 08:30 to 17:00hrs seven days a week and you will also work alongside Complex Care Nurses, Occupational Therapists, Physiotherapists, pharmacy technicians and students.
