Full Job Description

At Leeds Community Healthcare NHS Trust, we believe in providing exceptional care that goes beyond the ordinary, and we need someone like you to help us achieve that vision.

When you join us, we will work with you to develop your own personal development plan that supports your learning style. We provide training, clinical supervision and access to a peer support network.

This role is crucial in supporting the Registered Nurses on an evening to enable patients to remain at home and avoid hospital admission.

You will support the Registered Nurse, responding to intervention call outs between the hours of 5pm and 10pm. You may also work alongside a colleague and visit patients to deliver planned care.

You must have excellent communication, organisational and IT skills.

It would be expected that you are available to work on a rota over 7 evenings per week, 365 days per year. Your hours of work will be 17:00pm - 22:00pm. The role is based from Hunslet Health Centre, though it is expected that you travel across the city of Leeds during your shift to deliver patient care.

The contract is for a minimum of 10 hours (2 evenings) up to a maximum of 30 hours (6 evenings). Hours can be discussed prior to application or at interview as required.

Please note we are unable to offer sponsorship for this job role.

You must have a full driving licence and access to a vehicle.

Working for Leeds Community Healthcare NHS Trust, Leeds Community Healthcare provides a range of community-based health services across the whole city. We treat people in their homes, local health centres and community hospitals, and work in partnership with other local Trusts, charities and Leeds City Council. We have a strong culture based on deeply-held values, and our focus at all times is on delivering high quality care to the people of Leeds. In our most recent CQC evaluation, we were rated "Good", with our Adult Services rated "Outstanding" for caring.
