Community Nurse - Gloucester

To work as a member of the Community Nursing Team within an Integrated Community team (ICT) delivering care to patients/ residents within an Integrated Care Team population. To provide a high standard of skilled nursing care to people in their own homes or clinic settings. To manage the Community Nursing Team and Nursing Caseload in the absence of the team Leader - District Nurse / Senior Community Nurse (for annual leave, study leave & sickness).Main duties of the job Assess patient and carers needs through strength based assessment.Develop individual care plans in partnership with patients and carers. This will include information, advice, sign-posting and education. Provide skilled nursing care interventions, support and advice to colleagues, patients and carers.Collaborate with colleagues in the Integrated Community Teams and other aligned services to enable people within their communities, maximising their potential and promoting their independence. Administer and monitor medications consistent with policies and procedures. Assist in providing consistently high standards of evidence based care that meets the patient's requirements and satisfaction. Maintain accurate patient documentation on the electronic clinical recording system.

This high-level overview shows we are in a healthy position, with higher scores than average for comparable organisations, alongside a great response rate, indicating good staff engagement. However, we also know we have plenty of room for improvement in many areas. To that end, we continue to prioritise and invest in our commitment to genuinely becoming a Great Place to Work with consistent top-quartile performance in the annual staff survey and Pulse surveys.
