You will work closely with patients, families, GPs, and multidisciplinary colleagues to assess needs, plan and deliver care, promote independence, and support positive health outcomes. Part of your responsibilities include monitoring patient progress, managing a caseload, maintaining accurate patient records, delivering evidence-based care, promoting health and independence, participating in team meetings, and contributing to quality improvement initiatives.
