Community Nurse

Are you passionate about making a meaningful impact to the lives of young people with autism and their families? Were looking for a Band 5 Nurse to join our CAMHS Autism Intensive Support Team in Kirklees.

This role will provide intensive support to complex young people and families at risk of placement breakdown or hospital admission. Working with an MDT to assess needs, develop personalized care plans, and implement effective interventions. The team aim to reduce the risks of a hospital admission or placement breakdown, alongside improving the overall wellbeing of the young person.

All employees of the Trust are strongly encouraged to have their up-to-date flu vaccination to protect staff and patients., We are looking for someone who is enthusiastic about working with young people with complex presentations and who is keen to develop their skills and knowledge in working with autistic children and young people. We will provide opportunities for training and joint working with all members of the MDT. This role will have a particular focus on identifying and supporting the health needs of young people.
