Full Job Description

An exciting opportunity has arisen for a Senior Community Nurse to join our friendly and supportive Community Nursing Team. In this role, you will provide expert nursing care to patients in their own homes, supporting individuals with complex health needs, long-term conditions, frailty and end-of-life care.

Working autonomously and as part of a multidisciplinary team, you will undertake comprehensive assessments, develop and evaluate person-centred care plans, and coordinate care to help patients remain safely at home. You will play a key role in admission avoidance, helping to facilitate timely hospital discharge, and promoting independence and self-management.

As a senior member of the team, you will provide clinical leadership, support service development, and contribute to the supervision and development of newly qualified nurses through structured preceptorship and mentorship.

This is an excellent opportunity to make a meaningful difference to patient outcomes by delivering high-quality community-based care where it matters most.

To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by assessing, planning, implementing and evaluating packages of care. To facilitate/support newly Qualified Nurses in their Preceptorship year.

· To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.

· To work within the integrated team to facilitate early discharge from hospital

· To work within the integrated team to prevent unnecessary admission to hospital

· To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
