Newly qualified nurses will benefit from a comprehensive Preceptorship Programme to aid in their professional development within the community care sector., As a Community Nurse, you will provide high quality nursing care to patients within their own homes and community settings. You will assess, plan, implement and evaluate care, ensuring patients receive safe, effective and compassionate support throughout their healthcare journey. Working both independently and as part of a wider multidisciplinary team, you will build strong relationships with patients, families and carers, helping them manage their health and wellbeing while promoting independence wherever possible. Person SpecificationEssential
• Registered Nurse with a valid NMC registration
• Passion for delivering high quality, patient-centered care
• Strong communication and interpersonal skills
• Good organizational skills and the ability to manage a varied workload
• Ability to work independently and as part of a multidisciplinary team
• Flexible and adaptable approach to work
• Confidence using IT systems, including Microsoft Office applications
• Full UK driving license and access to a vehicle for work purposes

• Previous experience within a community, district nursing or primary care setting
• Experience managing patients with long-term conditions or complex care needs
