Full Job Description

Working with your community nursing team, you'll help deliver safe and person-centred care that supports people to stay well at home.
You'll be part of an integrated service that provides high-quality nursing care to adults with a wide range of needs. Your work will help people manage their health, stay independent and feel supported in their own homes and residential settings.
You will join a friendly and committed team who work closely with colleagues across the Integrated Nursing and Conditions services. We provide a supportive environment where you can grow your clinical skills and contribute to continuous improvement., What you'll do:
+ Carry out holistic assessments and develop clear, person-centred care plans
+ Deliver safe and effective nursing care in line with each person's needs
+ Work with colleagues and partners to support joined-up community care
+ Use and update the Electronic Patient Record (EPR) to maintain accurate notes

+ A registered nurse with current NMC registration
+ Good assessment, communication and clinical skills
+ Ability to work well in a team and build positive relationships
+ An interest in community nursing; you may be newly qualified or experienced
+ A willingness to learn, develop and adapt to changing needs
