An exciting opportunity has come up to join our busy Neighbourhood Team. We are looking for enthusiastic, hardworking, Band 5 Community Nurses to work 30 or 22.5 hours per week. Community Nursing is an integral part of the NHS 5-year plan with an emphasis now being placed on admission prevention. You will need to be passionate about community services and shaping a new way of delivering care. We are an integrated team who work collaboratively and compassionately with therapists, local authority services, the urgent care response team and our colleagues from The Primary Care Network to provide patient centred, responsive care which meets the complex and varying needs of our housebound community. We have dedicated time for study and clinical supervision and the support of a learning and development team for guidance with career progression., Your typical working day will include home visits for adults over 18 years needing a range of nursing interventions, including, but not limited to end-of-life care, all types of wound care, IV therapy, PICC line care, catheter care. You will be responsible for holistic assessments of patients taking into consideration their environment and social circumstances. You will support in the education of student nurses, both pre and post registration, as well as nurse associates and health care assistants. As a band 5 nurse, you will oversee patients and assist in decision making regarding their care. You will report to your immediate Band 6-line manager and have the support of the wider leadership team. You will also attend daily handovers and participate in Multi-disciplinary Team meetings.
The ability to communicate effectively at all levels Flexible working to provide care across the local area (the service operates from 7:30am 6:30pm, Monday - Sunday incl bank holidays) An enthusiastic, motivated and innovative thinker. The ability to work autonomously, manage your own case load and work within a multi-disciplinary team The ability to problem solve Experience of working in a fast paced and varied environment Full UK driving licence and use of car.
