Full Job Description

We have a rewarding opportunity available for a Community Navigator to join the team within our Peterborough Navigators Service.

Our Community Navigator Team offer person centred support to clients to help them live well and achieve their individual outcomes. The team use their local knowledge to link people in with services and use their knowledge and expertise to provide advice, information, guidance and signposting., As the Community Navigator, your role will aim to address the rising needs of older adults in Cambridgeshire by connecting them with local services, community groups, and emotional support to reduce isolation, enhance mental health, and support independent living.

Candidates must have an NVQ level 2 (or equivalent) in Health and Social Care or willingness to work towards this.

Previous experience of working with individuals to assess their needs and produce plans to meet outcomes is essential for this role.

Salary: £14.94 per hour

Hours: 16 weekly
