Full Job Description
We are recruiting a Community Mental Health Support Worker to join our Primary Care Network (PCN) team in South Tyneside, working as part of the Enhanced Primary Care Hub based at the CNTW NHS Bedewell Unit in Hebburn.This is a permanent, part-time role, contracted at 22.5 hours week. This is a hybrid role This role is not eligible for sponsorship as is does not meet the requirements as set out by the UK Visa and Immigration Service. About the roleIn this role, you will provide one-to-one, recovery-focused support to adults living in South Tyneside who are experiencing mental health challenges. You will use your skills in active listening, empathy, and person-centred practice to help individuals identify their needs, develop goals, and work towards meaningful change.Support is delivered predominantly face-to-face at Bedewell, with limited telephone contact. Sessions are typically weekly in the first month and then move to fortnightly depending on individual need.You will play a key role
in supporting people to maintain hope, build resilience, and improve their overall wellbeing, while ensuring services are delivered in line with our values of compassion, accountability, respect, excellence, and innovation.What you'll doProvide 1:1 recovery-focused support to individuals with mental health needsHelp people develop coping strategies, build resilience, and maintain hopeSupport engagement in community activities and the development of social networksPromote independence and informed decision-makingSignpost and connect individuals to appropriate services and community resourcesUse a range of communication methods (face-to-face, phone, text, email)Develop and maintain knowledge of local pathways and support optionsSupport the development of sustainable peer and community support networksManage caseloads effectively, ensuring accurate and timely record keepingWork in a culturally sensitive, inclusive way, challenging stigma and discriminationSupport a diverse range of service
