Full Job Description
Are you a passionate Mental Health Nurse, Social Worker or Occupational Therapist looking to make a meaningful difference in people's lives? Do you thrive on helping individuals build on their strengths, achieve their recovery goals, and live well within their communities?, In this role, you'll support adults experiencing severe and enduring mental health difficulties, providing high-quality assessments, therapeutic interventions, and personalised recovery planning. You'll work alongside a wide network of health, social care, voluntary sector and community partners, helping individuals access the right support at the right time. This is a fantastic opportunity to further develop your clinical expertise, contribute to service improvement, and play an influential role within a supportive multidisciplinary team. You must be a car driver with access to transport for work purposes. Community mental health experience would be advantageous., As a Mental Health Practitioner, you will:
- Manage a varied caseload of adults with complex mental health needs within the community.
- Complete comprehensive mental health and risk assessments using a strengths-based, recovery-focused approach.
- Develop, implement and review personalised recovery and risk management plans.
- Deliver evidence-based psychosocial interventions that promote independence, wellbeing and resilience.
- Build effective therapeutic relationships with service users, families and carers.
- Work collaboratively with colleagues across health, social care, primary care and voluntary sector organisations.
- Support individuals through periods of crisis, relapse and recovery while promoting positive outcomes.
- Act as an advocate for service users, ensuring their voice remains central to care planning and decision-making.
- Contribute to clinical leadership within the team through supervision, mentoring and supporting less experienced colleagues where appropriate.
- Participate in service development, quality improvement and clinical governance initiatives.
