Community Mental Health Practitioner

You'll work alongside a wide network of health, social care, voluntary sector and community partners, helping individuals access the right support at the right time. This is a fantastic opportunity to further develop your clinical expertise, contribute to service improvement, and play an influential role within a supportive multidisciplinary team. You must be a car driver with access to transport for work purposes. Community mental health experience would be advantageous., As a Mental Health Practitioner, you will:

• Manage a varied caseload of adults with complex mental health needs within the community.
• Complete comprehensive mental health and risk assessments using a strengths-based, recovery-focused approach.
• Develop, implement and review personalised recovery and risk management plans.
• Deliver evidence-based psychosocial interventions that promote independence, wellbeing and resilience.
• Build effective therapeutic relationships with service users, families and carers.
• Work collaboratively with colleagues across health, social care, primary care and voluntary sector organisations.
• Support individuals through periods of crisis, relapse and recovery while promoting positive outcomes.
• Act as an advocate for service users, ensuring their voice remains central to care planning and decision-making.
• Contribute to clinical leadership within the team through supervision, mentoring and supporting less experienced colleagues where appropriate.
• Participate in service development, quality improvement and clinical governance initiatives.
