Full Job Description
This is a 6-month fixed-term contract within the team, working across GP surgeries to support individuals aged 18 and over with severe and enduring mental health conditions, as well as those presenting with trauma and complex needs.
The post holder will play a key role in providing advice and triage, carrying out clinical assessments where appropriate, and offering short-term interventions for individuals experiencing complex mental health difficulties. Where required, you will facilitate referrals and transitions into the most suitable services within the transformed Community Mental Health Service., We currently have a vacancy for a full time Band 5 Community Mental Health Practitioner within our HMR Neighborhood Mental Health Team.
Please see JD for full information- please note this refers to Care coordination but this role is working within NMHT. We are a service that works with adult's age 18 years and upwards.
This is a full time post working core hours of Monday to Friday, condensed hours over 4 days will be considered.PARIS training would be an advantage however training will be provided if required.You will be working as part of a very experienced team and this opportunity would make a good pathway for anyone wishing to pursue or develop a career in community mental health.
