Full Job Description

The post holder will work as a key member of the Living Well Team, delivering the Short Term Offer (STO) to individuals experiencing mental health difficulties. The role focuses on providing timely, person-centred, recovery-oriented interventions to support individuals to stabilise, build resilience, and access appropriate ongoing support where required. The practitioner will undertake holistic assessments, develop collaborative care plans, and deliver brief therapeutic interventions aimed at promoting independence, reducing risk, and preventing escalation or crisis. Working within a multidisciplinary team, the post holder will contribute to a flexible, responsive service that promotes early intervention, maximises service user choice, and strengthens links with wider community resources.,
- Undertake comprehensive, person-centred mental health assessments, including risk assessment and management.
- Deliver evidence-based, short-term interventions in line with STO principles (e.g. psychoeducation, brief therapeutic approaches, signposting).
- Develop and review individualised care and safety plans collaboratively with service users and their networks.
- Promote recovery, self-management, and independence, supporting individuals to achieve their personal goals.
- Work in partnership with carers, families, and other professionals to ensure coordinated care.
- Facilitate timely access to appropriate services, including stepping up or stepping down care as required.
- Maintain accurate, timely clinical records in line with organisational and professional standards.
- Contribute to multidisciplinary team discussions, case reviews, and service development.
- Identify safeguarding concerns and act in accordance with local and national policies.
