Full Job Description

We are a specialised countywide Dementia Home Treatment Service that provides support to the person living with a dementia and their families, carers and loved ones. We build relationships, problem solve, listen, assess multiple care needs, direct plans of care, provide signposting, advice and education. We are seeking highly motivated individuals who are passionate about working with individuals living with dementia, promoting the concept of living well with dementia. You will need to hold excellent communication skills, be enthusiastic, flexible in your approach and demonstrate an understanding of the complexities involved in this specialist area. As part of our team, you will be working autonomously and have a varied role allowing plenty of scope to both demonstrate and develop your skills. You will work closely with and be supported by our multi-disciplinary team. Whilst working alongside the immediate team you will also work with our wider health and social care community, in particular our colleagues in general practice, care homes, social care, admiral nurses and our 3rdsector services such as the Alzheimer's Society. This role requires frequent travel within a wide geographical area to support service delivery across a wide geographical area spanning Shropshire, Telford and Wrekin. 1 vacancy will be based in Shrewsbury and the other in Oswestry.,

  • Take responsibility with supervision and support from a Band 6 Nurse, for the management of an identified caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
  • Maintainaneffectivereportingsystembyobservingandreportingverballyandin writingon patientconditions.
  • Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
  • Support in the provision of formal and informal training of pre/post registration students, staff members, carers and service users.
  • Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
