Community Mental Health Nurse

Band 6 Community Mental Health Nurse We are looking for a skilled and motivated Registered Mental Health Nurse to join our Community Mental Health Wellness Team at NELFT. This role offers the opportunity to work autonomously within a supportive multidisciplinary team, delivering high-quality, recovery-focused care to adults with complex mental health needs. You will act as a named key worker, playing a central role in assessment, care planning, and coordination of care to improve outcomes and support independence., Key Responsibilities Act as a named key worker for a defined caseload, coordinating care and supporting recovery-focused outcomes Undertake comprehensive mental health assessments and develop, implement, and review care plans Deliver evidence-based interventions for individuals with complex and co-existing needs Safely administer, monitor, and review medication in line with NMC and Trust standards Assess and manage clinical risk, escalating concerns appropriately and contributing to risk management processes Work collaboratively with GPs, primary care, and partner agencies to support safe discharge and continuity of care Maintain accurate, timely clinical records to support care delivery and audit requirements
