Full Job Description
This post will involve working with older people with complex mental health needs. You will be expected to support both the client and their carers. You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults., Clinical
- To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
- To hold a case-load as Lead practitioner and attend relevant MDT meetings
- To regularly support and cover the duty rota
- To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
- To provide risk screening and assessment as well as risk management planning
- To contribute to and facilitate evidence-based assessments and individual and group interventions
- To contribute to the maintenance and development of the Service
- To regularly share and supervise students on practice placement, and junior staff as appropriate
The successful candidate will join a friendly, highly-skilled and enthusiastic multi professional team. We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives. You will be part of a highly-skilled and supportive multi-disciplinary team based in Worthing. You will receive regular managerial and clinical supervision. This is challenging work but we receive very positive feedback from our service users and carers and Sussex Partnership NHS Foundation Trust has recently achieved outstanding for the care it provides. The post holder will be expected to travel across the geographical area ( Adur, Arun & Worthing) covered by the team, the ability to travel to remote areas is essential., - Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
- Completion of Mentorship Course/ENB equivalent
- Evidence of continuous professional development, Significant post qualifying experience in the relevant care group
- Experience of working consultatively with professionals
- Experience of conducting clinical assessments including risk assessments
- Experience of partnership working within and across various statutory and non statutory teams and agencies
- Experience of working within the CPA process
- Understanding of clinical governance
- Previous experience of working in the community
- Contribute to the implementation of services, Successful candidates are normally appointed to the bottom of the pay scale unless previous NHS experience in a similar role can be evidenced., You must have appropriate UK professional registration.
