Full Job Description

HMR Older People's CMHT is the community mental health team for older people. It is a borough wide team and provides a service to Rochdale, Middleton and Heywood. The team is comprised of community mental health nurses, an occupational therapist, team manager and an administrator.
Role Summary:
To undertake the role of care co-ordinator in the assessment, treatment, and delivery of care to older people with mental health problems, who meet the criteria for access to the service.
You will ensure that the highest quality care is carried out in accordance with the policies and procedures of Pennine Care NHS Foundation Trust and the Nursing and Midwifery Council., To undertake holistic mental health and risk assessments using trust approved documentation and procedures.
To manage a caseload of clients who will have a variety of mental health conditions, both organic and functional.
To provide evidence based therapeutic interventions promoting enablement, choice, and recovery.
To comply with all legislative requirements in relation to service users care.
The post holder will support the duty system that is operated within the team and respond to crisis calls they receive.
Actively participate in multi-disciplinary meetings and contribute to clinical decision making.
Engage and supervise student nurses.
This role does not involve commissioning services on behalf of the local authority., Sponsorship Information - We are an approved sponsoring organisation. If you require sponsorship, we will assess your eligibility based on current government guidance. If the role is not eligible and you cannot demonstrate your right to work, your conditional offer may be withdrawn.
Once Checks Are Complete -
The Recruitment Team will coordinate with you and the hiring manager to confirm a start date. You'll be booked onto a Trust Welcome Session and receive your Pennine Care NHS Terms and Conditions.
