Full Job Description
Most importantly, you must demonstrate care, compassion, and a strong commitment to working with older adults who experience severe and enduring mental health condition.
Through the role of care coordinator / lead professional, you will manage a caseload of around 25 people who present with complex needs and comorbidities including psychological, physiological and social care needs alongside positive risk management.
Demonstrating patience, motivation, flexibility, adaptability, and intuition will enable you to deliver exceptional care while upholding our established standards.
As part of our MDT and the wider healthcare network, you will provide holistic, patientcentred care in various community settings. This includes assessing clinical needs, assessing and managing risk, developing care plans, and delivering effective interventions.
You will be willing to support our team to deliver 1:1 and group-based interventions such as depot administration, medication monitoring, psychosocial interventions and structured clinical management.
Working as part of our MDT, you will support learners, colleagues and the wider team in care delivery, service improvement and team development.
You will work within professional, legal and ethical frameworks, roles and responsibilities.
