Full Job Description

We are recruiting a Community Matron in Westminster to cover maternity leave. For the right candidate, this is a real opportunity to shape patient care and put nursing right at the centre of community services.   • Co-ordinate care provision through a case management approach for a defined high risk patient group with complex long-term conditions with the aim to prevent hospital admissions and facilitate timely discharge.
  • Participate in MDTs including general practice and care homes MDTs, integrated neighbourhood meetings, high-intensity user forums within the acute sector.
  • Provide a high standard of clinical care: using advanced skills, expert knowledge to instigate and provide clinical treatments based on evidence-based practice.
  • Participate in the delivery of educational programmes to patients, carers, health and social care workers that promotes self-care principles.
  • Develop and promote robust MDT working across the wider Health and Social care sectors.
  • Monitor the quality-of-care provision, identify and promote areas for service development.
  • Act as named Community Matron for a Primary Care Network, providing support and care coordination for the wider health team within the network.
  • Work proactively with District Nursing colleagues and providing links across community nursing.
  • Provide clinical leadership and support to end of life pathways, including the UCP framework.
