Full Job Description

The post holder will be based within the Learning Disability and Forensic directorate. Working closely with the Learning disability Service Manager to review the current business activities within the service. The post holder will also be the lead in promoting the service, identifying good practice, and working closely with the team to implement any recommendations. The post holder will have direct line management responsibilities for the agreed service and robust management of the team. The community team manager will assist and oversee the day to day operational responsibilities for the clinical services within the community team and the wider multi-disciplinary team., Key aspects of the role include but are not limited to: 1) Person-centred care / Coordination of care This post will work alongside relevant clinical leads to ensure the safety and quality of a person's journey by effective planning and co-ordination of the episode of care, including the smooth transition to other settings. 2) Leadership and Staff Management This post will act as a visible leader within the team that enables professional decision making and effective team working. This post will lead and participate in annual staff appraisal, ensuring staff performance and conduct issues are dealt with under appropriate policy/procedure. Other roles include duty rotas/annual leave and study leave to ensure adequate staffing cover and appropriate skill mix for the team. 3) Operational Management This role will ensure appropriate delegation to nurses/AHP's and support workers and monitoring of outcomes for effective and safe care/service delivery. Managerial and organisational skills are needed to ensure that products required for service user care/treatment are procured in timely fashion and demonstrate value for money. 4) Service Improvement This role will work in partnership with a range of clinicians and managers in the planning and development of the service promoting the involvement of service users and carers.
