Community Learning Disability Nurse (Full-time | Part-time)

Are you a newly qualified Learning Disabilities Nurse? Do you want to work with a diverse set of clients?, We have 2 roles available a full-time role with our Abingdon based team and a part-time (22.5 hours per week) role with our Banbury based team. In this role, you will work collaboratively with individuals, families, carers, and partner agencies to assess health needs, plan and deliver care, and promote health and wellbeing. You will support service users to access mainstream healthcare services, reduce health inequalities, and maintain independence within their own homes and local communities. • Deliver safe, effective, and person-centred nursing care within the community setting
• Undertake holistic assessments and develop, implement, and evaluate care plans
• Promote health education and support individuals to make informed choices
• Work in partnership with families, carers, and other professionals
• Support access to primary and secondary healthcare services
• Participate in safeguarding processes and risk management
• Maintain accurate clinical records and contribute to service improvement
