Community Learning Disability Nurse

This is an exciting developmental opportunity under Annex 20 for a band 5 nurse to work towards band 6 competencies. Achieving band 6 competencies will enable you to transition into a band 6 role. It's a rare opportunity to join our friendly Community Learning Disability Team in North Tyneside. Our team is a large well established and supportive multi disciplinary team offering holistic health care to children and adults who have a learning disability living in North Tyneside. The Ideal candidate will have an understanding of National and Local Policies relating to the management of people with learning disabilities., The post holder will be welcomed as a member as a member of the friendly, community learning disabilities team in Northumbria Health Care Trust offering regular team meetings, robust support and supervision, and a commitment to continued professional development. As an integral part of North Tyneside Community Learning Disability Team, the post holder will be solely responsible for a personal caseload including assessment, planning, implementing and evaluation of care. This post is based in our Primary Care Workstream and the post holders primary role will be promoting health inequalities and supporting patients to access primary care. the post holder will support the dementia pathway and help develop and deliver training to a variety of people supporting the health needs of people who have a learning disability.They will be passionate about working in collaboration with service users, their family, carers and professional colleagues and will support patients and the wider workstreams within CLDT to enhance health and wellbeing, to prevent or reduce behaviour that challenges and improve mental health outcomes.,

• To provide a high quality community nursing service to adults and children who have a learning disability within the North Tyneside area.
• To work across Primary and secondary healthcare services to ensure equality of access for those with learning disabilities.
• To work with other agencies and services (statutory and non statutory) to ensure equality of access for those with a learning disability and facilitate effective communication between clients and other agencies.
• To provide community nursing support to adults and children who have behavioural/mental health problems. Support the design and implement Positive Behaviour Support Plans to provide intensive, proactive support and crisis intervention to patients, carers and care providers to prevent/ reduce admission to in-patient units and to maintain community placements.

• Mentorship Qualification or relevant equivalent qualification (essential requirement for post, expectation this will be completed within an 18-month period)
• R.N.M.H/R.N.L.D. Qualification, Evidence of ongoing professional development
• Experience working with people who present with challenging behaviours and experience of working within a PBS framework.
• Experience of multi-agency working.
• Experience working with children and adults
• Experience working in the community setting
• Evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period)., Knowledge of or experience in coaching and mentoring practices and tools.
• Knowledge of or experience in Quality improvement tools, techniques and methods., It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role
