Full Job Description

We are looking for a highly motivated and compassionate Community Learning Disability Nurse to join our multidisciplinary Learning Disability Team. This is an exciting opportunity to deliver specialist, health interventions to adults with learning disabilities, supporting them to achieve better health outcomes and improved quality of life. You will work across a variety of settings, including people's homes, day services and health settings, providing holistic specialist health assessments and interventions to adults with complex health needs., Undertake comprehensive specialist assessments and develop personalised care plans Lead and contribute to clinical pathways including physical health, positive behaviour support, autism, epilepsy and dementia Work collaboratively with multidisciplinary teams, families, carers, and partner agencies Promote health equality and wellbeing, reducing health inequalities for people with learning disabilities Manage and prioritise your own caseload, ensuring safe and effective care delivery Provide training, supervision, and mentorship to colleagues, students and support staff Act as a clinical resource, offering specialist advice and guidance within the team
