Full Job Description
The post holder is responsible for the specialist assessment of client needs, including those with complex needs and/or behaviours that challenge, the development, implementation and evaluation of programmes of care. The post holder will have responsibility for a caseload. The post holder will also act as a health facilitator for people with a learning disability.
The role involves close collaborative working with multi-disciplinary colleagues in the community setting, managing own caseload, Supervising others, completing assessments, raising awareness of the health needs of adults who have a learning disability, identifying unmet health needs and providing specialist nursing advice. The successful candidate will need to be proactive, self-motivated and have experience of autonomous working, whilst also working under the direction of the community team manager. The successful candidate will also be able to use current best practice to gain the best outcomes for Clients and their Carers. Will also assist in improving links with primary care services including hospitals/ G.P surgeries, provide support, information and education to clients, carers and provider's and also deliver education to people with learning disabilities around health needs both on 1:1 or group basis. Essential Requirements include: Substantial post registration experience as a Band 5/6 nurse working with people with a learning disability RNLD and current NMC Registration Excellent communication skills Experience of working with people with a learning disability in the community Driver with access to independent means of transport
