Community Heart Failure Specialist Nurse

We're a small, close knit, and supportive team who pride ourselves on working collaboratively and looking after one another.

We work closely with a wider multidisciplinary team, including cardiology consultants and a specialist pharmacist, to provide seamless, high quality care.

Currently, we cover the Calderdale community, with each team member spending one day per week reviewing inpatients remotely across CRH and HRI., You will bring confidence in cardiology, with a sound understanding of heart failure and its management. We are committed to supporting your development and offer a wide range of learning opportunities, including:

o Heart Failure Diplomao Independent Non Medical Prescribingo Advanced patient assessmento A variety of additional courses tailored to your learning needsYou will work autonomously, managing your own time and caseload to deliver high quality care to patients with heart failure. This includes:o Initial assessmento Medication optimisationo Ongoing education and support to promote self management

One day per week will be dedicated to reviewing inpatients at Calderdale Royal Hospital (CRH) and Huddersfield Royal Infirmary (HRI) (currently completed remotely).

As our Hospital at Home service develops, the team will provide care 7 days a week, 8am-8pm, and you'll have the exciting opportunity to help shape and influence the future direction of this service.

You will receive a supernumerary period and ongoing support from an established Heart Failure Specialist Nurse, ensuring you feel confident and supported every step of the way.
