Full Job Description
We are looking for a caring, motivated Healthcare Support Worker to join our friendly multidisciplinary team, providing care to patients in their own homes and residential care settings. The Role You will: Carry out independent visits to patients in the community. Deliver high-quality, person-centred care. Work closely with nurses, therapists and other members of the multidisciplinary team. Monitor and report changes in patients' conditions., As a Band 3 Healthcare Support Worker within the team, you will provide high-quality, person-centred care to patients in their own homes, supporting them to maintain their independence and wellbeing. Working under the direction of registered nurses, you will assist with personal care, monitoring and recording observations, wound care, catheter care, simple dressings, and other delegated clinical tasks. You will identify and report changes in patients' conditions, maintain accurate clinical records, and contribute to care planning. The role involves travelling between patients' homes, working collaboratively with the wider multidisciplinary team, and promoting dignity, respect and patient choice always. You will also support health promotion, infection prevention and ensure equipment is used safely in line with Trust policies.
Ability to work independently and as part of a team. Good communication and organisational skills. Ability to meet the travel requirements of the role and willingness to travel across the Droitwich, Ombersley and Rural patch. Working Hours This is a 7-day service with shifts starting at 7:30 am, 9:00 am or 10:30 am. The latest finish time is 6:30 pm, including weekend and bank holiday working on a rota basis.
