Community Health and Social Care Support Worker

We're looking for caring, dedicated Support Workers to join our Community Support Bedded Unit in the High Peak.

We have flexible opportunities available, with contracts ranging from 22.5 to 37.5 hours per week. You'll work across a 24-hour, 7-day service, including weekends and Bank Holidays, on a variety of shifts including early, afternoon, and night shifts.

Our unit is located in the High Peak, where public transport links are limited, so applicants should be able to travel to and from the service reliably.

As a Community Health and Social Care Support Worker, you'll play a vital role in delivering high quality health and social care services to people across Derbyshire, helping individuals maintain their independence and achieve the best possible outcomes.

Working alongside our registered healthcare professionals, you'll support the delivery of both clinical and non-clinical care. Depending on the service, you'll work under the direct or indirect supervision of a registered practitioner, Unit Manager, Deputy Manager, or Senior Support Worker, carrying out a range of healthcare and therapeutic tasks appropriate to your role.

You'll provide safe and compassionate care, always working within your level of competence and following established policies, procedures and clinical standards.

Whether supporting people in their own homes or within our Community Support Bedded Unit, you'll be part of an integrated team providing coordinated health and care services. Your contribution will help ensure people receive the right care, at the right time, in the most appropriate setting, promoting recovery, independence and wellbeing.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
