Be the First Point of Support That Makes a Difference We are looking for motivated and customer-focused Community Health Navigators to join our BSW (Bath & North East Somerset, Swindon and Wiltshire) Community Health Services Single Point of Access (SPA) team. As a Community Health Navigator, you will play a vital role in helping patients access the right care and support at the right time. Acting as the first point of contact for patients, members of the public, healthcare professionals and colleagues, you will provide a professional, compassionate and efficient service across a range of community health services. Working within a busy and supportive team, you will handle enquiries via telephone, email and post, providing information, guidance and signposting to appropriate services. You will also be responsible for processing referrals, maintaining accurate patient records, and ensuring patients are directed to the most appropriate service in line with established pathways and
protocols., As a Community Health Navigator, you will act as the first point of contact for service users accessing the BSW Community Health Services Single Point of Access (SPA). You will handle incoming calls with empathy and professionalism, assess information, determine urgency, and direct enquiries appropriately. Your responsibilities include managing sensitive and complex information confidentially, working closely with clinical colleagues, accurately updating patient records, processing referrals and correspondence, booking, cancelling, and rescheduling clinical appointments, and ensuring waiting-time targets are met. Additionally, you will identify and escalate urgent or incomplete referrals to the appropriate healthcare professionals and provide administrative support to clinicians and managers, contributing to service priorities as needed. The role requires excellent communication and organisational skills, strong attention to detail, and the ability to work confidently with sensitive and
confidential information.
