Full Job Description

We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Assistant who is keen to deliver a range of quality care services to patients visited by the Rural Alliance Care Community team based at Chowley Oak Tattenhall.

Applicants MUST demonstrate how they will meet the travel requirement for this post.

The hours are full time 37.5 per week. The role involves visiting patients in their own homes to provide nursing care which includes, venepuncture, wound dressings, care of diabetics and end of life care.

As part of a friendly team, you should have experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and have experience of providing care for palliative patients. This post involves lone working and requires travel across the Western Cheshire footprint, access to transport for work purposes is essential and MUST be demonstrated in your application.
