Full Job Description

  • Support the Team Leader in delivering a high-quality specialist Community Diabetes Nursing Service, contributing to service planning and delivery in line with Trust objectives.
  • Provide expert clinical assessment, management, and review of patients with diabetes, ensuring safe, evidence-based, and patient-centred care.
  • Work collaboratively as part of the multidisciplinary team, liaising with primary, secondary, and community care services to ensure coordinated care pathways.
  • Lead and support the development, implementation, and evaluation of specialist nursing interventions and service improvements.
  • Use project management and clinical leadership skills to contribute to service development aligned with national guidance, local priorities, and best practice.
  • Maintain accurate, timely, and comprehensive clinical documentation and records in line with professional and organisational standards.
  • Contribute to the development and review of clinical guidelines, policies, and procedures, ensuring high standards of care within the service.

    An exciting opportunity has arisen for an experienced Band 7 Community Diabetes Specialist Nurse to join the Community Diabetes Service in Harrow. The post is based at Honeypot Lane Health Centre and involves working within a well-established multidisciplinary team to deliver high-quality, evidence-based diabetes care across community settings.
  • The successful candidate will provide specialist clinical expertise to support adults living with diabetes, promoting effective self-management and improving patient outcomes. The role requires both autonomous practice and close collaborative working with primary care, community services, and secondary care colleagues. In addition, the post holder will contribute to service development initiatives, deliver structured education to patients and healthcare professionals, and support the ongoing improvement and transformation of community diabetes services within Harrow., We are proud to be one of the largest community healthcare providers in the country, with more than 4,500 colleagues caring for over four million people across London and Hertfordshire.
