Full Job Description

Based at St Mark's Hospital and King Edward VII Hospital, Windsor, we are looking for a compassionate and motivated Community Continence Assessor to join our community nursing team. This rewarding role involves visiting patients in their own homes who are unable to attend clinic appointments. You will carry out continence assessments, undertake abdominal examinations, identify continence-related issues, complete relevant documentation including fluid input and output records, and recommend appropriate continence products to support patients' individual needs. The successful candidate will have excellent communication and interpersonal skills, be able to work independently, and demonstrate a patient-centred approach to care. You will work closely with patients and colleagues to ensure high-quality assessment and support is provided. A full UK driving licence and access to your own vehicle are essential, as regular travel to patients' homes is required and some locations are not accessible by public transport.   • Assist the District Nursing Sister in achieving a high standard of care that promotes the health and wellbeing of patients in their own home.
  • Prioritise workload appropriately with due regards to the requirements of patients/ carers and the District Nursing Teams.
  • Be responsible for ordering and maintaining stock and continence equipment.
  • Work as a member of the District Nursing Team, contributing to the overall work of the team within the boundaries of their role and attend meetings as required.
