Community Care Officer Level 2

We are delighted to offer three full time posts for the role of Community Care Officer Level 2 (CCO) within our Lincolnshire Conversation Team. The areas we cover are: Lincoln, Gainsborough, Hykeham and surrounding villages.

This is a fixed term contract running to The positions have arisen due to internal progression and development within the team.

A Community Care Officer level 2 (CCO) plays a vital role in supporting adults with care and support needs, ensuring they receive the right support to live independently. This role is central to delivering statutory duties under the Care Act 2014.

About the role

As part of the Lincolnshire Conversation team, you will work with people who require guidance and support. Our goal is to support people to achieve their aspirations, access their communities, and promote independence. Working in a person-centred, strengths-based approach, you will complete Adult Needs Assessments and Reviews in line with the Care Act, focusing on what individuals can do rather than what they cannot do.

You must have the ability to travel across the county within reasonable timeframes in order to meet the needs of the service. This may include responding to unplanned visits at short notice, which could involve travel outside the local area or county. Mileage expenses are reimbursed for travel from the office to appointments.
