Are you passionate about delivering outstanding care and leading teams to make a real difference? We're looking for an experienced Community Care Lead to support our complex care services and ensure exceptional outcomes for our clients., OneCall24 Healthcare provides specialist, person-centred complex care to children and adults in their own homes, communities, workplaces and educational settings, working with the NHS, local authorities, case managers and privately funded clients.
About the RoleAs a Community Care Lead, you will provide operational leadership to care teams, oversee client care packages, and ensure services are delivered safely, effectively, and in line with regulatory standards. Working closely with operational, clinical, recruitment, compliance, and scheduling teams, you will play a key role in maintaining high-quality, person-centred care.
Responsibilities Lead, support, and develop care staff through supervision, appraisals, and ongoing guidance.
Oversee client care plans, assessments, quality reviews, and service delivery.
Support workforce planning, rota management, and staff engagement.
Promote safeguarding, compliance, and best practice across the service.
Build strong relationships with clients, families, multidisciplinary teams, and commissioners., Delivering exceptional, person-centred care.
Supporting continuous improvement and professionalism.
Working with honesty, dedication, and teamwork.
Putting our clients at the heart of everything we do.

Requirements Minimum 1 year's experience in care coordination and a senior or supervisory care role.
Previous experience working within complex care, supported living, or a similar environment.
Strong knowledge of care delivery, rota planning, safeguarding, and regulatory standards.
Excellent communication, organisation, and people management skills.
Flexible, proactive, and willing to travel as required.
