Full Job Description

An opportunity has arisen for a Community Assessment Officer within Salford's Adult Contact Team. We are looking to recruit workers to help us deliver our community led support vision.

You will be responsible for supporting your manager in ensuring that adult's care and support needs are met through delivering high quality support. You will be the first point of contact for the person requesting support. You will undertake strength-based conversations with adults, supporting them to maximise their wellbeing. You will offer a person centered approach to ensure that a compassionate and personalised approach is offered to the person you are supporting. You will work closely with other organisations to ensure that people are well connected, this will include supporting people to access statutory or voluntary services within their local community.

The Adult Social Care workforce aims to improve outcomes for adults who live in Salford by empowering individuals, adopting a strength's-based approach across the service, promoting independence, and working in partnership with our residents so their experiences, opportunities and lives are as good as they can be. This will be achieved through developing a skilled and motivated workforce by investing in their continued professional development and ensuring their practice is current and of a high standard.

Main duties of the job

To maintain a high standard when carrying out statutory functions on behalf of Salford Adult Social Care, you will assess adults under the Care Act (2014).

Your role will include conducting a mixture of telephone assessments and home visits to gather information to complete strength-based assessments. You will consider any caring responsibilities the adult may have for a child. Your primary focus will be to prevent, reduce or delay the need for traditional social care services. Your focus will be connecting adults to their local communities and promoting their independence and well-being.

You will develop support plans with adults and their carers or families, and conduct reviews to ensure that their care and support needs are met whilst promoting their well-being. Participation in multi-disciplinary meetings with health, education, or other professionals will be required.

With appropriate support, you will undertake assessments of an individual's mental capacity, taking into account the specific decisions that need to be made. You will be responsible for ensuring the safety of adults and contribute towards safeguarding meetings when appropriate.
