Full Job Description

The post holder will work across East Lancashire NHS trust where multi-professional learning opportunities can be provided, supporting the development and delivery of a specialist multi-level communication skills programme and Advance Care Planning (ACP). Act as a role-model for excellent communication and provide expert knowledge/advice and support the delivery of the communication skills strategy, o Support the development and delivery of communication skills and ACP training to all grades of staff including pre and post registration students o Work with the clinical teams to support and promote advance care planning which requires management of sensitive and complex information o Support clinical staff in undertaking extremely complex and emotive conversations with patients and provide constructive feedback o Provide 1-1 support in clinical areas to staff undertaking difficult conversations who require understanding of complex communication strategies and models o Role model the use of effective communication skills to clinical staff and provide tailored information to support effective communication with patients, allowing their needs and wishes are understood and met as far as possible, for example, negotiating a plan of care at end of life. o Evaluate the impact of communication with both the patient and staff. o Act as a resource for communication skills and ACP for staff - (eg) support complex and emotional conversations relating to end of life, delivering bad news, managing strong emotions o Maintain accurate records of staff training delivery both formal and informal, requiring data input and analysis, ensuring key staff groups are prioritised for training, o Support the development and delivery of communication skills and ACP training to all grades of staff including pre and post registration students o Work with the clinical teams to support and promote advance care planning which requires management of sensitive and complex information o Support clinical staff in undertaking extremely complex and emotive conversations with patients and provide constructive feedback o Provide 1-1 support in clinical areas to staff undertaking difficult conversations who require understanding of complex communication strategies and models o Role model the use of effective communication skills to clinical staff and provide tailored information to support effective communication with patients, allowing their needs and wishes are understood and met as far as possible, for example, negotiating a plan of care at end of life. o Evaluate the impact of communication with both the patient and staff. o Promote good practice in communication and ACP o Act as a resource for communication skills and ACP for staff - (eg) support complex and emotional conversations relating to end of life, delivering bad news, managing strong emotions o Be involved in the continuing development and delivery of specialist training programmes to support the requirements of the ELHT workforce - this includes role modelling, presentation delivery and facilitation of groups o Maintain accurate records of staff training delivery both formal and informal, requiring data input and analysis, ensuring key staff groups are prioritised for training o Support the Communication Skills lead in providing verbal and written reports o Analyse reports complied by others to highlight themes relating to communication and advance care planning and the impact on patient care. o Manage time appropriately to achieve goals set in the Education strategy o Participate in the design, implementation and evaluation of current and/or new programmes of education and training, specific to communication skills and ACP. o Ensure facilities for training delivery are suitable and meet the requirements of the session o Develop skills in teaching and assessment to meet all learning styles o Implement policy for own area and propose changes o Respond to the changing requirements of staff training and be able to provide specialist advice to a wide range of professional services o Responsible for maintaining stock control and reordering of resources as required COMMUNICATION o Support working with external organisations o Act as a role model and support students and staff across ELHT o Support audit of education and clinical practice and, if required, make changes to training programmes and provide feedback, both verbal and written. o Using excellent communication to promote training o Build and maintain professional working relationships within the wider team o Utilise excellent communication and knowledge to facilitate advanced training sessions and manage challenging questions/feedback TRAINING AND DEVELOPMENT o The post holder will have experience of delivering/facilitating training sessions o Deliver/facilitate communication skills and ACP training to staff at all grades o Training requirements to be met by attending train the trainer courses where required and will play a key role in reflecting on their own and the practice of others. o Develop and maintain knowledge and competence in agreed, foundation and advanced communication and ACP training o Maintain own up-to-date knowledge base ensuring personal and academic credibility. Critically analysing own performance, inviting feedback and peer review. PROFESSIONAL RESPONSIBILITIES o The post holder will be a registered professional and will maintain their registration throughout. o Act in a professional manner at all times and adhere to the professional code of conduct at all times o Manage and prioritise own workload o Take responsibility for personal performance and recognise own development needs o Participate in an agreed personal development / appraisal scheme o Attend mandatory training relevant to the post as specified by the trust.

  • Please note that the salary advertised is for full time hours. If this post is less than 37.5 hours per week, the salary will be pro-rata. For posts on Agenda for Change pay-scales, new entrants to the NHS will normally commence on the first pay point of the relevant band.
