We have new opportunities within our All Age Continuing Care Specialist s117 Aftercare and Acquired Brain Injury Team. We are looking for clinical leaders with detailed knowledge in mental health and s117 aftercare, who are passionate about patient care and improving peoples life opportunities. You must share our leadership values of being compassionate, person centred, honest, innovative, and collaborative. Posts involve working across Hampshire, Southampton and Isle of Wight. As Clinical Team Leader (Mental Health Nurse, Learning Disabilities Nurse or Social Work registration with mental health specialism) you will be responsible for leading a team of professionals supporting clients who require care, treatment and support under the Mental Health Act, including overseeing comprehensive assessments of clients and championing a positive risk-taking approach alongside least restrictive practices and strengths-based approach., The successful candidate will be able to use specialist knowledge and clinical skills to ensure that coordination and outcomes are determined in a consistent manner to ensure effective application and interpretation of the MHA S117. Main responsibilities

  • Managing a team of professional staff and administrators, providing clinical supervision and professional expertise around complex s117 aftercare
  • Leading and contributing to the training and development of all staff
  • Screening and approve new funding applications and packages of care against least restrictive practice and strengths-based principles
  • Monitoring and actively managing the clinical caseload and decision-making
  • Participating in discharge planning meetings, Dialogue plus and Care and Treatment Reviews
  • Providing proactive case management to individual complex and rehabilitation clients

    You will have excellent leadership skills, able to motivate and support staff and lead compassionately. You will need extensive interpersonal and communication skills to establish good rapport with teams, stakeholders and clients and an ability to make decisions in a challenging but supportive environment. You will have the opportunity to develop strong links with health and social care colleagues to ensure effective multi-disciplinary working and develop a strong understanding of local NHS and independent sector service provision in individual's own homes and in care homes, Be accustomed to a high degree of autonomy on a day-to-day basis
  • Be able to assimilate large amounts of information and have excellent verbal and written communication skills, with attention to detail
  • Ability to develop relationships and foster effective networks with key stakeholders, families and carers
  • Be able to assess and prescribe care dependent on the individuals holistic health and care needs
  • Have excellent time management skills
  • Excellent knowledge of Microsoft office and have intermediate typing skills
  • Be able to commit to regular onsite working and able to travel daily across the county and occasionally countrywide

    £49,387 to £56,515 a year Please note for part time hours the salary will be pro rata
