Full Job Description

To provide Clinical Leadership and support the continued development of our CAMHS Home Treatment Service. The CAMHS Home Treatment Team will ensure a speedy, high quality and person-centred response to Young People presenting in Mental Health Crisis across the County and offer an alternative to admission. You will provide clinical risk and mental health assessment, formulation and short-term contingency planning for children and young people presenting in mental health crisis, you will also be involved in building relationships with our tier 4 inpatient unit to support early discharge and admission avoidance. Working within the community, you will deliver a range of brief solution focused interventions to empower patients, carers, families and colleagues in decision making and assist in identifying and referring on to a range of health and social care resources for support. You will work collaboratively with other Mental Health Services across the system as well as Acute Hospitals, Ambulance Service, the Police, Education and Social Care to provide risk and mental health assessment, formulation and contingency planning for children and young people presenting in Crisis.,

  • To undertake clinical mental health assessments and support the management of Children and Young People.
  • Ensure that care is based on the assessment of care needs and includes a thorough risk assessment and risk management plan.
  • Use clinical knowledge and expertise to formulate an assessment of relapse risk of the patient after specialist assessment. Liaise with other agencies caring for patients, including community mental health teams and GPs and use collateral information from relatives, GPs and past clinical reports, where available and appropriate, to provide care plans for all patients.
  • Provide Supervision and Clinical leadership within the Service
  • Deputise for the Team Manager when necessary
  • Utilise recent research findings in the advancement of best practice and in the delivery of an evidenced based service.
  • To provide support and expert advice to colleagues involved in the care of patients with mental health needs and develop systems to ensure that this support is available in a range of ways.
  • Monitor and evaluate the standards of care and take actions to ensure that a high standard of care is maintained. Contribute to Audit to ensure quality standards are met.
  • Identify and be responsive to the diverse cultural, social and spiritual needs of all patients.
  • Promote general health education and specifically mental health education to the client group.

  • As the Home Treatment Team is a county wide team the ability to drive is essential. The main office base is in the village of Fulbourn. The post holder would be expected to be flexible, with working in the office, across county and sometimes from home. This would be agreed through on-going discussion with the Team Manager.
